Plan for your first visit

1. We'll get in touch

Once your appointment is confirmed, we will email you an invitation to create a profile on OnPatient.com.
2. Create your profile

Once you create your profile on OnPatient, please answer the patient questionnaire before your first appointment. If you have any questions or concerns, feel free to reach out via phone or email.
3. Your first visit

Please arrive 15 minutes prior to your scheduled appointment and plan for a 90 minute exam. (Functional Medicine patients only).

We have a parking lot to the left and right of our building.

If you’re looking at the building, our office is on the far right end.
